Details of Procedure for Demand for Disclosure etc. of Personal Information

The details of procedure for Demand for Disclosure etc. of personal information are as follows.

1. How to Make a Request

(1) Send an e-mail to Privacy Consultation Desk (pms@septeni-holdings.co.jp) to receive a Demand for Disclosure etc. request form from the Company.
(2) Then please prepare and send the necessary documents specified on the next section by mail (registered mail).

(Note) Our office is unable to receive requests in person.

2. Necessary Documents, Etc.

(1) Request Form for Disclosure, etc. of Personal Information
Request form prescribed by the Company (Please complete all items inside the bold frame.)

(2) Identification documents
(i) If the Principal makes a request
Please submit one of the following documents stating the Principal’s name and current address as identification.
a. Copy of driver’s license (If there are changes to the Principal’s name or address, a copy of the back side is also required.)
b. Copy of passport (Copies of both the name and address page and the page containing the photo.)
c. Copy of residence card (If there are changes to the Principal’s name or address, a copy of the back side is also required.)
d. Copy of special permanent resident certificate (If there are changes to the Principal’s name or address, a copy of the back side is also required.)
e. Certificate of residence (Must be issued within the last three months and must not contain your Individual Number (“My Number”). Please note that photocopies are not accepted; only the original certificate issued by the municipality is accepted.)

(ii) If an agent for the Principal (the “Agent”) makes a request
a. One of the documents listed in (i) a. to e. above regarding the Principal (Identification document of the Principal)
b. One of the documents listed in (i) a. to e. above regarding the Agent (Identification document of the Agent)
c. Documents proving the Agent's authority:
- In the case of a statutory representative: A certified copy of the family register or a certificate of residence that includes both the Principal and the Agent and indicates their relationship. (Must be issued within the last three months. Information about the registered domicile is not required.)
Note: If you have already submitted a certificate of residence stating the relationship as document a. (Identification document of the Principal) or b. (Identification document of the Agent) above, no additional documents to prove the agency relationship are required.
- In the case of a voluntarily appointed agent: A power of attorney (signed by the Principal and affixed with the Principal’s registered personal seal (実印)) and a certificate of seal registration (印鑑証明書) for the seal used on the power of attorney.

(3) If notification of the purpose of utilizing Personal Information and disclosure of Personal Information is requested, a fee of 800 yen (including tax) needs to be paid.
 Please enclose a postal fixed-amount money order with the necessary documents.

4. Notes

(1) If the submitted documents have any deficiencies, such as missing items and blurred identification documents, we will return the submitted documents to the address stated on the request form.

(2) We may be unable to respond to a request in certain cases, such as if the response would violate laws or regulations.

(3) Our response to a Demand for Disclosure etc. of Personal Information will be sent in writing to the address of the requester (if the requester is an agent, the address of the agent) stated in the Demand for Disclosure etc. request form.

(4) The Demand for Disclosure etc. request form and other documents submitted for a Demand for Disclosure etc. of Personal Information will not be returned. We will store submitted request forms and other documents at the Company for the necessary periods and will then dispose of them. The same will apply when deletion of Personal Information is requested and the Company deletes the Personal Information.
